If you have a community care and support plan provided by your local authority, we will seek your consent to access it to understand your specific care and support needs. To describe our services and discuss how you prefer to receive your care and support, our management team will schedule a home visit. The manager will clarify the potential risks with you if any and determine the necessary actions to ensure safety. All these details will be documented in your personal care and support plan, of which a copy will be provided to you at your home. Only after you or your authorized representative have given written approval to the care and support plan. we will initiate the service. In rare emergency situations, we may need to provide immediate assistance. In such cases, a supervisor or other qualified individual will be sent to conduct an initial risk assessment and give the necessary care and support. Comprehensive risk assessments and care and support plans will be completed within 2 days of the service commencement. Making Changes to Your Care and Support Plan
During consulting, we routinely analyze your care and support plan, discuss with you about what is effective, and identify any necessary adjustments. Normally, this occurs every six months but may be done more frequently if required. Our care assistants are strictly bound to follow the guidelines in the care plan and are not authorized to perform any other or additional tasks. If you wish to modify your care and support plan, please contact our office at 01493 719010 to arrange a visit from our team member.
We will introduce your care assistant(s) to you before your service begins, whenever possible. This helps you and your care assistant(s) to know each other and will be familiar with your care and support plan. After each visit, they will document the care and support provided to you using an electronic care planning system or in your care and support plan folder. If you are dissatisfied, please inform us and we will take care of your concerns. If necessary, different arrangements will be made for you.All care assistants carry an identity badge and must present it to you for verification.
Care visit times are approximate and may vary by up to 30 minutes from the scheduled time. If your care assistant is late on time, please allow up to 30 minutes for potential traffic delays, etc. If the care assistant has not arrived after that time, please call us at 01493 719010 so that we can check on the situation and the safety of the care assistant.
If there is no electronic call monitoring system in place, the care assistant will request your signature on a timesheet after each visit. If you can't sign your name, we will do special arrangements for that too.
